Abstract
Background:
@EN While the effect of high dose chemotherapy for the treatment of various advanced solid cancers has been widely accepted, myelosuppression contributing to the increase of the infection was dose-limiting.
@ES Methods:
@EN To reduce the morbidity and mortality associated with infection in patients with grade IV neutropenia which was induced by chemotherapy, we employed 2§¶/kg/day of rhG-CSF for the neutropenic (neutrophil<1,000/§§) patients after chemotherapy.
We
evaluated 23 patients from June 1992 to August 1993.
@ES Results:
@EN Twenty three patients were evaluated; 5 with non-Hodgkin's lymphoma, 4 stomach cancer 4 small cell lung cancer, 4 sarcoma, 2 non-small cell lung cancer, 2 ovary cancer and 2 head&neck cancer. The median age was 58 (range, 15~75). The
interval
from
the first day of chemotherapy to the nadir of neutrophil count was 15.0¡¾1.6 days (range, 12~18 days). The mean platelet nadir count was 91.660¡¾60.880/§§. The mean duration of grade IV neutropenia was 3.1¡¾1.4 days (range, 1~6 days). The
neutrophil
recovery was 13% in 3 days after the start of rhG-CSF administration, 65% in 5 days, 96% in 7 days and 100% in 11 days. The mean duration of rhG-CSF administration was 4 days (range, 2~10 days). The incidence of neutropenic fever 57%. The
infection
source was identified in 2 patients (sepsis, pneumonia) and there was no death associated with 57%. The infection source was identified in 2 patients (sepsis, pneumonia) and there was no death associated with infection during the period of
neutropenia.
There was no serious side effect of rhG-CSF observed.
@ES Conclusion:
@EN With those results, we cold recognize that rhG-CSF (Neutrogin(r)) effectively decreased the duration of neutropenia and the mortality associated with infection after cytotoxic chemotherapy causing no serious side effect.
